From 1 June 2019 Primary Care Reimbursement Services (PCRS) will process all Long Term Illness (LTI) applications.
Patients and service users can apply by completing an application form and posting it to:

From 1 June local offices should send any applications forms to PCRS at the address above.
We will no longer provide the LTI Green Book. Instead, LTI patients will get an LTI card.
Patients using LTI books
Patients currently using LTI books will not have their books replaced. We will issue LTI cards as we update patients details.
A valid prescription must be available when dispensing products under the LTI scheme and be available for review to support an LTI claim.
An express registration process is available to pharmacies for patients diagnosed with specific long term illness, for example diabetes and epilepsy. In these case pharmacies can register the patient online. Further information will be sent directly to the pharmacies.
