Integrated care brings together the many specialities involved in patient care so that, from the patient’s perspective, the services delivered are consistent, personalised and coordinated.

Following the International Conference on Integrated care was held in Dublin in 2017, it is evident that progress in integrated care has been made through a series of initiatives and pilots. It is therefore a good period to assess nationwide preparedness for the delivery of integrated care and highlight success factors and barriers based on daily experiences.

The Clinical Strategy and Programmes Division is inviting all those providing care from both community and acute settings to participate in an anonymous survey to share their views on the preparedness of HSE services to deliver integrated care.
